1. Log Your Session While at the Gym:
   You must complete this form while at the gym, as your location will be verified. Submissions made after leaving will not count.
2. Earn Rewards Automatically:
   Each point you earn is worth $1 and is transferred to your gift balance on your Shopify account linked to your Skool email.
3. Stay Honest, Stay MASSive:
   To ensure fairness and community integrity, we may request proof of your workout through gym check-in records or time-stamped photos.
4. Use the App for a Seamless Experience:
   The easiest way to stay accountable is by using the MASS Monster Accountability App. Log your sessions quickly and stay on top of your progress!

Rules and Regulations for the Accountability Form System

1. Incentive Rules: Each incentive may have specific rules and offer different prizes. Please review the current incentive details for complete guidelines.
2. Point Transfers: Points earned through the Accountability Form will automatically transfer to the gift balance associated with your Shopify account linked to your Skool email. For any issues, contact support@massmonster.life.
3. Daily Point Limit: Members are eligible to earn 1 point per day. Submitting multiple forms within the same day will not result in additional points.
4. Verification Requests: In certain cases, members may be required to provide proof of workout. Acceptable forms of verification include: Online/app gym check-in records, and time-stamped workout photos.
5. Fraud Prevention: Points and gift balances may be revoked if fraudulent activity or misuse of the Accountability Form system is detected.
